Primary Care Physician Services in the home, including:

• Assessments of patient's medical needs

• Follow-up visits

• EKG & routine blood tests

• Peripheral bone density measurements to assess for osteoporosis

• Arrangements for skilled nursing, rehabilitation, and custodial care services

• Social service assessments and care plans

• Caregiver support

The purpose of the Home Visit Services program is to provide primary care continuity services for homebound patients. Our program coordinator screens all potential patients or their primary caregiver with a telephone interview. If a patient is indeed eligible for the program a medical appointment will be made.

Eligible patients may have a wide variety of medical problems which may include senile dementia, Alzheimer's disease, strokes, heart attacks, disability secondary to orthopedic problems such as fractures, complications of diabetes, hypertension, depression, Multiple Sclerosis, Parkinson's or other chronic illnesses causing disability.

Our primary physicians are licensed, board certified or board eligible.
