Inpatient Referrals/Referral Process & Tours

If your loved one is currently in a hospital or skilled care setting, ask the case manager at the facility to initiate a referral to SIUH

• Call our referral hotline at (718) 226-9460 or 1 (800) RX-REHAB
• Our community liaison nurse will request and review all medical and insurance information
• The Chairman / a physician specializing in Rehab Medicine will determine if the admission is appropriate
• If the candidate does not meet the criteria for inpatient rehab admission, alternatives will be discussed

• Medicaid
• Commercial insurances (insurance authorization may be required prior to admission)
• Financial Assistance (sliding payment scale)
