Central Assessment and Intake Services
Located across the street from Staten Island University Hospital's South campus, the Central Assessment & Intake Services Unit has as its goal the comprehensive evaluation of each individual to determine if an individual is in need of chemical dependency treatment. The final section of this biopsychosocial evaluation recommends, when appropriate, clinical interventions so as to break the cycle of dependency as well as the appropriate level of care for this treatment to begin. All level of care decisions are based on the Patient Placement Criteria defined by The American Society of Addiction Medicine (ASAM). If the appropriate services are not available at SIUH, a referral will be made to another provider. Assessments are carried out by Physicians Assistants & Chemical Dependency Counselors with many years of experience working with addicted individuals.
There may be occasions when an individual or family member is uncertain whether or not a problem exists with alcohol or other drugs. In such a case, an assessment is available for a nominal fee. A report summarizing the findings & any recommendations is mailed to the responsible party within 7 days contingent upon a proper Release of Information being signed by the individual. Federal confidentiality regulations 42 CFR Part 2 applies to all individuals (including minors). Please note that this service is generally NOT covered by insurance.
The Central Assessment & Intake Services Unit is located at 392 Seguine Avenue, Staten Island, NY. It is open Monday thru Friday from 8:00 a.m. until 5:00 p.m. & on Saturday from 8:00 a.m. until 4:00 p.m. It is recommended that you call in advance to schedule an appointment but walk-ins are also welcome.
