
Inpatient
Referrals to the inpatient brain injury rehabilitation program are accepted from physicians, case managers, discharge planners and social workers. Self-referrals from patients and/or family members may also be made. A call to our referral hotline at (718) 226-9460 initiates the screening process. Our community liaison nurse will request and review all pertinent medical and insurance information on a case by case basis, and present it to the medical director of the program, who will determine if the admission is appropriate. If the potential candidate is found not appropriate for inpatient admission, alternatives will be discussed with the patient's family member, social worker or primary physician. We encourage prospective patients and families to tour our facility, meet the staff and have any questions answered prior to admission.
Outpatient
Referrals to the outpatient brain injury rehabilitation program are accepted from physicians, case managers, social workers and discharge planners. Call (718) 226-6362 for an appointment with one of our physiatrists and an initial screening. If appropriate, an evaluation with the brain injury team will scheduled. Insurance
Many
commercial insurance companies, Medicare and Medicaid provide coverage for
our comprehensive services. A sliding payment scale is also available for those
who qualify.
